Here's the latest newsletter!

You'll notice that instead of summarizing the research articles in our own words (since we're not doctors) we'd let the researchers speak for themselves. The full abstracts are now added in place of our editorial summaries. A hyperlink to the article follows the abstract for those who want to read more.

It's important to us to provide our community evidence-based information, and we felt that this change was the best way to meet that goal. Unconscious bias can seep in even where the writer is doing their best to avoid it. This may make some of the articles a bit technical for those without a science background, but we hope that you will use this forum as a resource to ask questions and discuss the research you may not quite understand.

We welcome comments and suggestions in this area, and would like to know how YOU feel about this change to our newsletter!

Now available! Would love to know what you found most interesting, or what would be helpful in the future.

Great updates from Dr. Davis and the CMV crew discussing different vaccines that have human IBD implications. Another case of MAP in a dog, and a promising new antibiotic with activity against non-TB mycobacteria. Plus Dr. Kuenstner details interesting genetic profiles on a family with a variety of immune disorders. Lots happening!

This is one of those open questions where we need more high quality research. In my opinion (not as a doctor) patients usually start with 2 or 3 antibiotics and see how they do for a couple weeks at half strength. If there are no issues, another antibiotic is added or the others are ramped up. Anecdotally, it is sometimes at the ramp up period where people experience a return of CD symptoms or die off (they can be similar so it's hard to tell). Docs can then work with patients to drop one antibiotic to try to figure out which one is the issue if symptoms are very severe.

Also, some people will either have an allergic reaction to one antibiotic, which then needs to be discontinued. A couple of our community members have had a severe reaction to AMAT and they have also mentioned they are generally "sensitive" to medication. For this group, they may only need a half strength dose.

In my own case, I had pretty severe nausea that I'm guessing was caused by the clarithromycin. I almost could not keep it down. This went on for a couple of weeks and then I started to get better. It felt like I was in a terrible CD flare. My doctor told me to watch for symptoms of obstructions and CDiff, but as long as I could tolerate it and take it easy, to continue on the protocol because hopefully my symptoms were due to die off. I should also mention I started on a full strength regimen on Day 1 (1000mg clarithromycin, 600mg rifampin and 500mg levofloxicin.) Levo is not commonly used for AMAT anymore due to the FDA warning about fluoroquinolines.

Dr. Chamberlin answered some general AMAT questions a couple of years ago which may also be helpful:

The key is to keep in touch with your prescribing doc as you go through the first couple months and bring up any concerning symptoms.

Hi.

How do you determine which protocol is best for you? How do you determine which antibiotic is causing issues?

There are a variety of different ways doctors have used antibiotics to treat IBD and other chronic immune disorders. People often ask me: Which one is the best?

Without huge double blind trials, it's hard to know, but below are some resources that discuss a variety of research and options on this topic. Feel free to add your own experience of what worked/didn't work for you, and research articles discussing different antibiotic protocols.

Here's another great resources from Guys Hospital that discusses treatment of Crohn's (adult patients) with AMAT, and provides contact info.

Other than Dr. Sanderson, who is an adult doc, I don't really know any peds options. Did you try the option in Dublin I suggested via email?

Hi Julie,

I spoke to them on the phone and they asked me to email them which I did, but didn't hear back from them. I will contact them again on Monday. I am still pursuing that lead, but chasing other leads as well, just in case.
